We can provide practical health and wellbeing guidance, including information on accessing healthcare services, finding local doctors and pharmacies, and understanding how to manage any ongoing health needs while you’re away.
To help us provide the most relevant support, please complete a short questionnaire about your current health situation.
Your information is secure, confidential and managed only by Teladoc Health. It will not be shared with your employer.
Once you’ve completed the questionnaire, our team will review your responses and contact you by email or phone if any additional guidance or support may be helpful. If no further action is needed, we’ll send you an email to let you know.

For the primary member (employee)
If you’re relocating with your family, you’re welcome to register for your dependent child(ren) and answer questions on their behalf, however dependent children over the age of 16 must answer them themselves if they choose to complete the questionnaire.
For the accompanying family member (dependents)
If you are an accompanying family member over age 16 please register and answer your own questionnaire.
Each questionnaire takes an average 5 to 10 minutes to complete
Privacy Notice
In accordance with the European Privacy Regulation 2016/679, and other applicable regulations, TELADOC HEALTH INTERNATIONAL S.A.U. inform you that the data provided will become part of a file that is the responsibility of TELADOC HEALTH INTERNATIONAL S.A.U., in order to provide the requested service and manage the relationship between both parties. Providing your data is not compulsory but if we are not provided with it, we will not be able to provide you with the services. You expressly consent to the processing of your data, including health information. Your data will be kept during the period of legally established responsibility or as long as it is necessary for the management of the relationship.
Your personal data, including health information, will not be transmitted to third parties, except in those cases where it is strictly necessary to communicate it to medical and healthcare professionals in order for them to provide you with a better service. If necessary, your data can be transferred to other countries. T ELADOC HEALTH INTERNATIONAL has appointed a Data Protection Delegate whom you can contact by email at dataprotectionofficer@teladochealth.com or in writing to: Vía Augusta 252-260, 2nd floor, 08017 Barcelona, addressed to “Data Protection Officer”. In the event that the data provided refers to persons other than the communicating party, the latter must previously provide them, inform them and obtain the consent for the treatment of the aforementioned. In the case of minors or incapacitated, the individual who facilitates the information has done so with the consent of the father, mother or guardian. The user has the right to access, limitation, portability and complaint to the Spanish Data Protection Authority, deletion or omission, rectification, cancellation and opposition to the processing of data by e-mail at dataprotectionofficer@teladochealth.com or in writing to the registered office at: Vía Augusta 252-260, 2nd floor, 08017, Barcelona.
